Some edits to templates of appeal and req.

I think some of the fields that are autocreated when you make an appeal are unnecessary. In an appeal, this is the template:

Warcraft III username:
Realm:
Gamename:
Bot username:
Situation:
Why you should be unbanned:

I think Gamename and Bot Username should be removed; no one fills them in correctly anyways. And how does it help us if we know what bot the user was playing on when banned? And, the gamename is also mostly filled in wrong, and we can just look at the ban logs and instantly see.

This is the template for ban requests:

Violator's Warcraft III username:
Game name or map name:
Approximate date/time of game (alternatively, link to a Stats page):
Rule player violated:
Time of incidents (Note in-game timer or replay timer):
Any further thoughts:

I think "Approximate date/time of game (alternatively, link to a Stats page)" should be changed to just "Link to a Stats page". It doesn't even help to know the approximate time/date of game, but it does speed up the process greatly to have a stats page link.
