Any further thoughts:
Right before the very end of the game, at the critical push by sentinel on bottom tower, axe refuses to defend. I didn't notice until after I died and saw axe on patrol movement at edge of fountain. At least two enemy heroes were very low on hp, so his help could have made a huge difference.

I saw in dialogue that player "Scores." also took note of his refusal to defend, so apparently it was going on for at least two minutes. Ez ban, I imagine.
