After reviewing (replay timers):

Butcher revives at 51.23, doesn't leave fountain or defend till game end
Winter wyvern revives at 52.04, doesn't leave fountain or defend till game end
And i'd also include a 3rd person here, rylai since he dind't defend either at the same time (52.04 till game end)

So deadly_alliance@useast.battle.net , olddaysdota@europe.battle.net and epidemy@server.eurobattle.net should be banned for refusing to defend (2 raxes were still up)
