Is this a permanent ban? I don't see a timeframe. My team asked me to leave for the sake of the game. People literally wrote "leave we won't report", i replied "now ?" a few times and after unanimous yes I left, please check.

This previously happened under a different context but same reasoning. You were unbanned but you still chose to leave this game.

With all due respect, it was not exactly the same reasoning. In the previous case it was due to flaming towards me. In this case if you check the chat transcript, players actually wrote "Our only chance is for you to leave, leave we won't report", and after replying "now?" a few times and getting a unanimous yes I moved on. Honestly I didn't expect the ban, but now I get that the end result is the same, even if everyone approves.

Different context under the fact the first game you left was due to your teammate flaming you while this game, you left because you became a liability to your team.

Same reasoning due to the fact that you left both games when you shouldn't have.

The rule is to not leave if the game hasn't officially ended - which you acknowledged on your previous appeal.

Yes I understand that now. I somehow assumed that there wouldn't be any repercussions because it was a unanimous request, and I see that often in games... Was foolish, I get that there is just no exception and it shouldn't happen at all.

ps: I would appreciate some leniency if possible, I had a different understanding of the rule in light of the context, and I understand now that there is no exception. Thank you.

Just so we have it clarified, you do understand that you should not leave games regardless of what your teammates are doing to you, yes? The rule is quite clear so it's quite repetitive to reiterate it multiple times.
